Reviewed plan **4df028e2** against spec **9ccfaca6**, amendment **56756f09**, reviews R1–R5, dispositions and the current code.

| Earlier finding requiring judgment | Status | Why |
|---|---|---|
| R5-1 — matching-region canary discarded after direct GET | Resolved | Task 22 carries the record into one create/rechannel/ready decision, reused for environment binding. Tests cover all three paths beyond the list cap. |
| R1-17 — declined scratch/build/release requirements | Resolved; decline accepted | The final operator rule authorizes the socket-test arrangement; release and all five cross-build gates are covered. |

The other earlier findings were closed by R5; I found no regression in those fixes.

**New finding — P2: Task 12 suppresses the required creation log while assignment is off.**

**Plan:** [line 4246](/home/paulca/amadan.net/carlos/platform-nearest-region/docs/superpowers/plans/2026-10-02-nearest-region-default.md:4246).
**Code insertion point:** [server.go:3550](/home/paulca/amadan.net/carlos/platform-nearest-region/internal/console/server.go:3550).
**Requirement:** [spec §6.5, line 587](/home/paulca/amadan.net/carlos/platform-nearest-region/docs/superpowers/specs/2026-10-01-nearest-region-default-design.md:587).

The proposed `if s.appRegionsOn()` suppresses O8 for successful creations during the initial switch-off rollout, after assignment is disabled, and when the home is unnamed. Those creations still store `""`/`default`, but §6.5 requires one region line per app creation without that qualification.

The tests assert that unsuccessful claims produce **no** line; none asserts the required successful-creation line across these configurations. They therefore pass with this omission.

**Fix:** emit O8 unconditionally after the successful insert and lost-race check. Add cases for assignment on, off and on with an unnamed home, asserting exactly one line containing the stored region/source. Retain the failed-claim silence tests. Reuse O8 through the existing copy-review gate.

No additional findings in migration carry, publication/reconciliation predicates—including `CARLOS_POOLS=",tito"`—create precedence, interface declarations, compatibility, C-string gates or rollout ordering. The dependency choices have no identified blocker: [MaxMind v1.13.1 requires Go 1.21](https://raw.githubusercontent.com/oschwald/maxminddb-golang/v1.13.1/go.mod), and [IANA provides tzdata2026b and its signature](https://data.iana.org/time-zones/releases/).

This was static review. I did not independently reproduce the recorded red/green runs or cross-builds. No files changed; the worktree is clean.

**FIRST PROMPT:**
Independent, read-only round-6 review of the nearest-region implementation plan.

**WHAT:**
Check prior findings, spec coverage, interfaces, compatibility and execution gates.

**STATUS:**
Earlier findings resolved; one new P2 concerning creation logging.

**NEEDED:**
No additional user input.

**RECOMMENDED:**
Remove Task 12’s logging condition and add successful-creation log assertions.

**SESSION:**
Safe to close; no files changed or processes left running.

**Verdict: not ready to execute as written. Task 12 still omits §6.5’s required creation diagnostic when assignment is off.**
